Depending on availability, purchases may be made either directly through m-x-w.com or through the assistance of our stores or personal shopping associates. All aspects of your online ordering and/or purchase will be performed by or on behalf of MXW Forms L.L.C. (referred to here as “We” or “MXW”).
Online purchases from this section of the Site may be made by residents of the U.S., for delivery to clients in the mainland U.S.A. and Alaska. We are unable to deliver online purchases to other places at this time. Certain European countries are able to purchase with MXW, please contact us if you are an international customer for certain details at inform@mxwforms.com
Product Availability
“MXW” products available for purchase online are subject to availability. As necessary, we reserve the right to limit the quantity of products we supply; supply only part of an order or to divide up orders. We also reserve the right to alter the terms or duration of any special offers or sale promotion. We will inform you if we are unable to fill your order. Each client may make online purchases of up to four items per style, unless otherwise stated. Please note the following product availability descriptions:
Available now: Products are currently being held in stock and are ready to ship.
Available for Pre-order: Products are available for purchase. However, the items have not yet arrived in stock and therefore are not ready to be shipped. You can secure your purchase now but you will be billed only upon shipment of the merchandise.
Out of Stock: Products are currently unavailable, and will most likely not be available in the future.
Pricing and Payment Options
While we try to ensure that all prices on our Site are accurate, errors may occur. If we discover an error in the price of the merchandise you have ordered, we will give you the option of reconfirming your order at the correct price or cancelling it. If we are unable to contact you, we will treat the order as cancelled. Risk of loss or damage to products you order on this Site passes to you on delivery of the products, title passes once we have received payment in full for the products.
Prices displayed on the MXW Site are quoted in U.S. dollars. MXW accepts the following forms of payment for online purchases: Visa, Mastercard, and American Express
The transaction will be charged to your credit card only after we have verified your card details, received credit authorization, confirmed stock availability, and your order is ready to be shipped. Your credit card will not be charged at the time you submit your order.
Sales Tax
MXW charges state, local and county taxes on online transactions, as required by applicable laws. Please note that your on-screen transaction total reflects estimated tax.
Shipping and Delivery
For security reasons, MXW online purchases will require a signature upon delivery. In addition, purchases may not be shipped to P.O. boxes or APO/FPO addresses, which are not secured by UPS. All purchases are shipped via UPS. To estimate the total delivery time for your purchase, please allow one to two business days to process your order for shipment, in addition to the time indicated by your preferred shipping method (i.e. UPS next day, second-day, ground). Orders are processed and delivered Monday through Friday, excluding holidays. MXW cannot be responsible for unanticipated delays.
Tracking
A shipping confirmation with the tracking information is e-mailed to you once your purchase has been shipped. If you have questions about the delivery of your purchase, an MXW sales assistant will be pleased to assist you via e-mail at inform@mxwforms.com.
Return Policy for Online Purchases
MXW’s aim is to give customer satisfaction. For MXW online purchases, we will accept merchandise not worn, used or damaged and with security tag in tact for a full refund when accompanied by the original sales slip. Refunds may only be issued in the same form as payment. Full-priced merchandise must be returned within 30 days from date of delivery; however, sale merchandise must be returned within 15 days of delivery. Shipping costs are non-refundable.
Changes/Cancellations
If you wish to cancel or modify your order, please send an e-mail as soon as possible to inform@mxwforms.com. We will make every effort to accommodate your request; however, once a product has left our premises for shipping, the purchase cannot be cancelled or modified until the product has been returned to us. In this case, please refer to our return policy and return your purchase once you have received it. If you would like to purchase a different product, please visit our website or contact a MXW online sales assistant.

Applicable Law
The laws of the State of Utah govern the supply of products from this website, without giving effect to any principles of conflicts of laws. In connection with any litigation, without limiting our right to bring and action against you in any jurisdiction, you agree to submit to the exclusive jurisdiction of and venue in the state courts located in the County of Salt Lake and in the federal courts located in the District of Salt Lake.
